Request for Admission (RFA)

Request for Admission (RFA) forms are required to schedule an admission at our hospitals. The hospital will transfer the details from these forms into a waiting time register, to ensure the sickest patients are treated first. 

RFA options

Consultmed eRFA (Westmead)

A new electronic Request for Admission (eRFA) form is available for referrals to The Children's Hospital at Westmead via Consultmed. 

Submitting an eRFA is easy – complete and send the form securely online. Hospital admissions team will process the referral.

The digital form streamlines admissions, cuts down paperwork, and ensures timely communication between specialists and admissions staff.

An eRFA for SCH clinicians will be provide by Consultmed at a later date.

SCHN PowerChart

This method requires an NSW Health account as well as a SCHN PowerChart account.

Paper form

Reach out to the booking office to have them send the relevant RFA form and/or confirm the preferred method of sending these forms to the booking office.
