Keeping your own record

At Sydney Children's Hospital, Randwick, parents and older patients may keep track of their own health care using a Patient Held Record.

What is it?

The Patient Held Record (PHR) was designed to help parents or patients keep track of their health care and to keep all relevant information together. The PHR was created in response to requests from parents of chronic children, many of whom had designed their own method of record keeping and had relayed their child's medical history to hundreds of health care professionals over the years.

How to use it

This Patient Held Record may be downloaded from the Sydney Children's Hospital, Randwick website, printed and placed in a ring binder. Additional pages may be printed as required and added to the binder. Test results and discharge summaries may be added in full in the appropriate sections.

This PHR is not part of your official medical records — it is for you to keep and look after.

This PHR is not for our health care professionals to write in, but it should provide a basis for discussion if any changes happen before your next visit, that you want to tell us about.
