HEEADSSS interview for adolescents
HEEADSSS is a widely used psychosocial assessment framework for adolescents that supports health professionals to explore a young person’s life in a structured, sensitive and non-judgemental way.
The framework covers Home, Education/Employment, Eating/Exercise, Activities/Peers, Drugs, Sexuality, Suicide/Depression, and Safety, helping clinicians identify both strengths and areas of risk that may impact wellbeing.
Importantly, HEEADSSS creates opportunities to identify emerging or unmet risks early and respond in a timely, supportive way. This may include addressing concerns within the consultation, strengthening protective factors, providing education, or linking the young person with additional supports. When used as an ongoing, collaborative conversation, the assessment supports prevention, early intervention and continuity of care.
Used well, HEEADSSS is more than a checklist. It is a guide for building rapport and gaining a holistic understanding of a young person’s mental health, lifestyle, relationships, safety and overall wellbeing, including the influence of digital and social media.
Building rapport
Start by creating a safe, respectful space where young people feel heard and valued. Opening the conversation with strengths-focused, everyday topics can reduce anxiety, build trust and support honest discussion.
Examples include:
- “To help me get to know you, what’s something you’re proud of?”
- “How would your friends describe you?”
While many young people are comfortable progressing through the HEEADSSS domains in order, flexibility is key. Let the young person’s priorities, cues and context guide the flow of the conversation.
For some adolescents, areas such as home or family dynamics may feel more challenging than topics like relationships or sexuality. Adjust your approach accordingly and move at their pace.
Focus on strengths
Actively identify strengths, protective factors and sources of support. Recognising what a young person is doing well highlights resilience and provides a foundation for collaborative problem-solving, goal-setting and self-management.
