Referral pathway

Trapeze accepts referrals for young people living with a complex chronic condition aged 16-25 years who have received care from The Children’s Hospital at Westmead (CHW) or Sydney Children’s Hospital, Randwick (SCH).

Eligibility

Not all young people with chronic conditions require Trapeze support. The service will best support young people who: 

  • are aged 16–25 years 
  • have multiple complex chronic health condition(s) and/or disability 
  • have an unclear health care transition pathway and require complex transition coordination 
  • currently receive care through Sydney Children’s Hospitals Network 
  • would benefit from support with transition planning, care coordination, or building independence in managing your health. 

Referral information for health professionals

Our department is using Consultmed to manage referrals. The Consultmed platform allows you to send e-referrals to us securely. Create an account to get started.

Please provide:

  • specific transition issues
  • medical conditions which require transition
  • details of adult services the young person been referred to if available

The referring doctor will receive an email from us confirming  the patient's eligibility for the service. We'll work closely with patient's care team to support transition planning to adult healthcare.
