Ankle Foot Orthosis (AFO) factsheet

Introduction

An ankle foot orthosis (AFO) is a device that supports a child’s ankle and foot.

AFOs are custom-made for each child using moulded plastic and metal.

They are used to:

  • keep the ankle stable
  • control foot movement
  • support weak muscles and unstable joints
  • help with walking and mobility
  • prevent differences in how the ankle and foot grow
  • reduce pain.

Fitting an AFO

AFOs are fitted by a physiotherapist or orthotist. Your child may need to wear them during the day to support their walking and movement.

Put on a long sock with few or no seams. Pull tight to avoid wrinkles and make sure it is comfortable.

Bend the knee and stretch the foot upwards as far as possible, as shown by your child’s therapist.

Place the heel firmly into the back of the AFO and check that it is in place.

Keep the foot in position and fasten the ankle strap firmly.

Fasten the top strap next, and the toe strap last (if there is one).

Straps should feel snug, not too tight or loose. You should not be able to slide a finger underneath the strap, and your child should feel comfortable.

To put on a shoe, loosen the laces and open the shoe up completely. Remove the insole to make more space if needed.

Slide the shoe on from the front of the foot, then push it back towards the heel while wriggling it side to side.

 Care of the equipment

AFOs should be cleaned regularly and checked for signs of wear-and-tear. 

If they do not fit well, they can cause pressure injuries.
