Trapeze is a service within the Youth & Transition Department at the Sydney Children’s Hospitals Network. We support young people as they move from children’s services into adult healthcare. 

Our team includes medical, nursing and allied health staff who understand how the health system works—and how overwhelming change can feel. We help you build the skills to manage your health, understand your care, and navigate adult services with confidence. 

You’re more than your condition. That’s why we talk with you about life beyond hospital appointments—things like home, school, work, relationships and lifestyle—because they all affect your health and wellbeing. 

We listen to what matters to you, take your goals seriously, and work alongside you at your pace. Trapeze is here to support you as you take the next step into adult healthcare.
