Referral pathway

Trapeze accepts referrals for young people living with a complex chronic condition aged 14-25 years who have received care from The Children’s Hospital at Westmead (CHW) or Sydney Children’s Hospital, Randwick (SCH).

Eligibility

Not all young people with chronic conditions require Trapeze support. The service will best support  young people 

  • who are not on treatment or attending clinic appointments
  • with frequent unplanned hospital admissions
  • who are Aboriginal and Torres Strait Islander
  • from culturally and linguistically diverse backgrounds
  • who require complex transition coordination
  • with complex psychosocial issues such as financial hardship, family problems, social isolation, unemployment or school absenteeism. 

Referral information for health professionals

Our department is using Consultmed to manage referrals. The Consultmed platform allows you to send e-referrals to us securely. Create an account to get started.

Please provide:

  • specific transition issues
  • medical conditions which require transition
  • details of adult services the young person been referred to if available

The referring doctor will receive an email from us confirming  the patient's eligibility for the service. We'll work closely with patient's care team to support transition planning to adult healthcare.
