Orthotics NDIS information sheet

Thank you for choosing us as your National Disability Insurance Scheme (NDIS) Orthotics service provider. We offer a professional and comprehensive service designed to help you achieve your goals.  

This information sheet explains our services, policies, and your rights and responsibilities. We look forward to supporting you in reaching your goals and making the most of your NDIS funding.

Our staff are happy to answer your questions and encourage you to contact us with your feedback.

About the Orthotics NDIS service

The NDIS Service Agreement

We will prepare a Service Agreement for you to sign which explains how your NDIS funding is to be used by us and what services you want us to provide, based on the goals in your child’s NDIS plan.

We will sign the Service Agreement and ask you to sign it too. We will give you a copy of the signed Service Agreement.

The Service Agreement is also saved to your child’s electronic medical record.

If you don't want a copy of the Service Agreement, we will note this in your child’s electronic medical record.  

If you find that the service agreement is not meeting your accessibility needs than please contact us. We have an electronic version and an easy-to-read version to accommodate your needs. This will gives you all the information and rights as the original document.   

Waiting lists

We require a referral from the appropriate hospital staff and/ or external specialists associated with the Sydney Children’s Hospital Network. 

Your child will be placed on our wait list based on our priority system. They will be seen as soon as an opportunity is available.  

You will receive either a letter or a phone call with your appointment date and time. You will also receive a text message reminder. Staff may also contact you via text message if you prefer.

What to bring to the appointment

Please bring any items that your child uses for day-to-day activities including:

  • AFO’s and any other devices
  • Relevant Equipment eg. walking frame
  • Shorts  
  • Long socks  
  • Relevant recent X-rays  
  • Snacks
  • Drink bottle
  • Child distraction/comfort toys 

Your child's treatment plan

The clinician will ask you about your child’s needs and goals.

In discussion with you, we will come up with a support or treatment plan that works for your child and home life.

Your clinician will monitor your child’s progress over time.

If your child’s needs or circumstances change at any time, please let us know. We will work with you to adjust the support plan. 
 

Information and privacy

We keep a record of your information to ensure you are provided with a quality service and to show that we meet our safeguarding responsibilities.

Your child will be automatically enrolled in the NDIS Quality and Safeguards Commission that collects participants demographic information for audit purposes.  

You can choose to opt out of this automatic enrolment by ticking the box on your child’s service agreement.
Opting out will not effect the service you are given. 

Your information will be kept in a confidential and secure manner.

You are entitled to request access to health information that is held in NSW Health services. 

You may be asked to apply for access in writing and provide identification. They may be charged a fee. 

Your rights and responsibilities

The Sydney Children's Hospitals Network is committed to supporting the rights of children, families and visitors.  

Understanding the rights of children and young people, their families and carers is vital to ensuring SCHN has a safe and professional hospital environment.  

We endorse the Australian Charter of Healthcare Rights and the Rights of Children and Young People in Healthcare Services in Australia .   

These rights apply to all people in all places where health care is provided in Australia.  

The charters provide a framework to help empower children and young people, and their families and carers to understand what they can expect from the staff caring for them. 

Fees and cancellations

Schedule of NDIS Service Fees  

The service fees charged will be at the latest NDIS price guide rate and relevant support category. 

These fees can be claimed via the NDIS Portal or by invoice to your Plan Manager, or your, if you are self-managing your NDIS plan.

Cancellations  

If you are unable to attend your appointment, please call us.

Please do not attend your appointment if you or your child are unwell.  

Withdrawing services

We will talk to you before we withdraw any support. Reasons why support may need to stop:  

  • Failure by Participants to adhere to the terms of their service agreement
  • Failure by Participants to adhere to the provider’s policies and procedures
  • Failure by Participants to communicate and provide information about changes to their care needs
  • Failure to adhere to workplace health and safety requirements
  • Failure by Participants to pay for services.

You are free to access your orthotic service with us or elsewhere. If you wish to see a different orthotist, please reach out to our department managers or reception staff. If you wish to see a different provider, please let us know so we can cancel your service agreement.

  • Listen to your views about your child’s care and answer any questions you may have 
  • Respect the privacy and confidentiality of your child and your family 
  • Ensure that you have a say in decisions about your child’s treatment 
  • Inform you of opportunities to be involved in research and teaching.

Advocacy  

There are a range of advocacy services that can help you speak up and get your needs met. Advocacy organisations can support you to make sure you are not discriminated against and are treated equally as well as respectfully by others.

If you need further help or information finding a disability advocacy service, please ask your clinician.
