Electronic Request For Admission (eRFA)

The eRFA form provides Staff Specialists and Visting Medical Officers (VMOs) the ability to electronically submit requests for inpatients.

An inpatient is a patient who has been admitted under their care at either The Children's Hospital at Westmead (CHW) or Sydney Children's Hospital, Randwick (SCH).

The completed form is then submitted electronically to the applicable CHW or SCH Booking Office for processing in real time. Health Staff receive an acknowledgement of their submission and a copy of the form via email.

This will assist in achieving a more efficient booking workflow, provides a greener approach to the environment and most importantly, direct information enhancing the patient health care journey.

The eRFA form is only available to those users who have been given access.
