About our service

The Kids Guided Personalised Service (KidsGPS) is the hospital service supporting children with complex medical conditions and their families coordinate their healthcare journey. 

The goal of our team is to make this as easy as possible for your child and your family by improving how all the services work together. 

The KidsGPS Care Coordinator will work closely with you to plan your child’s care and provide support as the main point of contact.  

Care coordinators work as health care guides, connecting the different services and empowering you to advocate for your child’s care. They will work with you to decide how long you may need the support for. 

What the service includes

Team communication: Ensuring clear communication among everyone involved in your child’s care and giving you better access to information about their treatment

Understanding the health care system: Guiding you on how to navigate the health and social care system and understand your role in it  

Creating a Care Plan with you, if appropriate

Helping children stay home: Supporting you in keeping your child at home by connecting you with your local healthcare teams and planning for common health concerns, which can help avoid trips to the Emergency Department (ED) and hospital stays 

Appointment bookings: Coordinating medical appointments, moving them closer to home, and setting up virtual care to save travel time if appropriate 

Transport and accommodation: Assisting with arranging transport and accommodation for medical appointments or procedures.

Accessing the service

KidsGPS Care Coordination supports the most medically complex children and their families.

You can reach out to the team directly or ask your child’s local doctor or paediatrician to refer them on. Referrals can be made via phone or email to the team during business hours.
