What does Trapeze do?

Trapeze is part of the Youth & Transition service and supports young people who are transitioning from the Sydney Children’s Hospitals Network to the adult health care system. We're made up of medical, nursing and allied health staff who know how the health system works. 

We'll work with you to develop skills to manage your chronic conditions, help you navigate the adult system  and empower you to take charge of your health. 

We understand you are more than your condition so we will ask about the other aspects in your life like home, school, work, lifestyle choices and relationships, because these all have an impact on your health. 

We will try our best to understand your needs and listen to you. We'll be your trapeze into adult healthcare!

Am I eligible?

We provide transition support to young people who meet the following criteria: 

  • 14-25 years old
  • Living with a complex chronic illness currently treated at SCHN, and
  • Have an unclear health transition pathway. 

I'm eligible! What do I do next?

You can be referred to Trapeze through your GP. Just ask your GP to send your e-referral securely through the Consultmed platform. 

In the 'Reason for referral' section, the GP should specify:

  • What are the specific transition issues?
  • What are the conditions which require transition?
  • Has the young person been referred to adult services? If yes, provide details. 

Your GP will receive an email from us confirming your eligibility for the service. We'll work closely with your team to support you in your transition planning. 

What if I'm not a patient with you?

No dramas! You can connect with an ACI Transition Care Coordinator (TCCs) near you. 

There are three ACI Transition Care Coordinators (TCCs) based in NSW adult tertiary hospitals.

They accept referrals for young people with chronic conditions 14-25 years who have not been treated at SCHN.
