Our services

Youth & Transition are a team of medical, nursing and allied health staff who provide holistic healthcare that supports young people as partners in their own care.

Our team works across three services:

We help young people to develop skills to manage their chronic health conditions and/or disability, navigate their transition from paediatric to adult health care services, and help empower young people to take charge of their health.

The Youth & Transition Arts Program

The Youth & Transition Arts Program empowers and promotes holistic health-care access, utilising arts practice and projects to optimise health and wellbeing for young people living with chronic health conditions and disability.

This youth-led program works with clinical teams to find creative ways to communicate health care information, enhance health literacy, support young people to build skills in self-management and independence and improve access to services.

Handbook for patients and families

We've put together a range of resources to help patients and families manage conditions better as they transition to adult health care services. Some young people find dealing with the health system frustrating and confusing, the Trapeze handbook for young people and families can help make it easier for you.
