Our services

Youth & Transition are a team of medical, nursing and allied health staff who provide holistic healthcare that supports young people as partners in their own care.

Our team works across three services:

We help young people to develop skills to manage their chronic health conditions and/or disability, navigate their transition from paediatric to adult health care services, and help empower young people to take charge of their health.

Handbook for patients and families

We've put together a range of resources to help patients and families manage conditions better as they transition to adult health care services. Some young people find dealing with the health system frustrating and confusing, the Trapeze handbook for young people and families can help make it easier for you.

Our Aboriginal Hospital Liaison and Education officers play an important role in the care and welfare of and advocacy for Aboriginal patients and families, helping ensure a responsive and respectful patient journey.

We can arrange a free interpreter for you. Interpreters are available for over 50 languages. Tell us which language you speak when your child is admitted, or let your nurse know.
