Youth & Transition are a team of medical, nursing and allied health staff who provide holistic healthcare that supports young people as partners in their own care. Youth & Transition is also responsible for our Trapeze and ChIPs services.

A new podcast created by four young people from the Chronic Illness Peer Support (ChIPS) program, Chronic Conundrums hopes to lend a helping hand in navigating the challenges that come with chronic illness and disability diagnoses for young people.
