Transitioning to adult healthcare

Young people who are transitioning from the Sydney Children’s Hospitals Network to the adult health care system are one of our priority populations. We aim to support and facilitate specialised care for clinicians who have patients who are transitioning to adult healthcare.

Our services

Trapeze is our health transition service supporting young people transitioning to the adult health care system.
Our Chronic Illness Peer Support (ChIPS) program is a service run by young people for young people aged 14-25 years.

Biography
The Transition Care Network works with clinicians, young people, families and carers to improve continuity of care for young people with chronic health conditions as they move from paediatric to adult health services.
