What does Trapeze do?

Trapeze is a service within the Youth & Transition Department at Sydney Children’s Hospitals Network. We support young people as they move from children’s services into adult healthcare. 

Our team includes medical, nursing and allied health staff who understand how the health system works—and how overwhelming change can feel. We help you build the skills to manage your health, understand your care, and navigate adult services with confidence. 

You’re more than your condition. That’s why we talk with you about life beyond hospital appointments—things like home, school, work, relationships and lifestyle—because they all affect your health and wellbeing. 

We listen to what matters to you, take your goals seriously, and work alongside you at your pace. Trapeze is here to support you as you take the next step into adult healthcare.

Trapeze referrals

Am I eligible?

We provide transition support to young people who meet the following criteria: 

  • 16-25 years old
  • Living with a complex chronic illness currently treated at SCHN, and
  • Have an unclear health transition pathway. 

I'm eligible! What do I do next?

You can be referred to Trapeze through your GP. Just ask your GP to send your e-referral securely through the Consultmed platform. 

In the 'Reason for referral' section, your GP should include:

  • the specific transition support you need
  • the health conditions requiring transition to adult care
  • whether you have already been referred to adult services (and if so, which ones). 

Your GP will receive an email from us confirming your eligibility for the service. We'll work closely with your team to support you in your transition planning. 

What if I'm not a patient with you?

No problem! You can link in with a Transition Care Coordinator (TCC) in your local area. 

TCCs are based across NSW, including at ACI, Hunter New England and South Western Sydney. They support young people aged 14–25 with chronic health conditions who have not received care through SCHN.
